The cause of the blood clots is due to the combination of estrogen and drospirenone (a synthetic progestin hormone) contained within Yaz contraceptive pills. Studies have shown that drospirenone causes a higher risk of developing blood clots than other hormones such as levonogestrel.
The blood clots created by using Yaz form deep in veins in the legs, arms or pelvis. This is known as a deep vein thrombosis, or a DVT. If these blockages are left untreated they can break away and flow with the bloodstream towards the lungs or heart. Migrating blood clots are extremely dangerous, and can cause Yaz pulmonary embolism, stroke or even a heart attack.

Doppler Ultrasonography
Ultrasound is described as the use of high-frequency sound waves that generate moving images of your blood vessels by bouncing between internal structures. Modern ultrasound techniques use M-mode (typically used to study the heart) and two-dimensional ultrasonography. The more modern approach is Doppler ultrasonography.
Doppler ultrasound is used to determine the direction of the blood flow as well as the velocity. This information is used to help aid physicians in identifying abnormalities such as valvular problems and venous compressibility. Both of these circumstances suggest that there is a presence of blood clots.

D-Dimer Testing: Uses And Limitations
A d-Dimer test measures the level of fibrin particles in your bloodstream. Fibrin is a byproduct of your body's normal clot-dissolving function. Higher levels increase the probability of blood clots, which could lead to a heart attack or stroke.
There are however limitations of a d-Dimer test. It is nonspecific, and therefore inconclusive. While a low to average level of fibrin fragments can conclusively exclude DVT, a high level merely suggests the possibility of a DVT – but does not imply the presence of one. High levels of fibrin can be brought on by pregnancy, infection, liver disease or a number of other leading factors.

Venography: The Gold Standard For Yaz Blood Clots
When using venography a contrast dye must be injected into the bloodstream to highlight any suspected blood vessels that could have a blood clot in them. The dye shows up on X-ray images of the veins. The doctor looks over the X-rays to identify any veins where the normal blood flow is obstructed or slowed. These symptoms denote the presence of a clot.
Doppler ultrasound has pretty much replaced venography as the main test for diagnosing DVT and Yaz blood clots. Ultrasonography is very accurate and noninvasive, and this is why it has become the popular method of diagnosis. These days venography is generally performed when an ultrasound is unable to definitively diagnose DVT, but the suspicion of blood clots remains high.

A Typical Testing Path For Yaz DVT
Ultrasonography is the first test typically done to diagnose deep vein thrombosis (DVT). If the probability of blood clots is small a d-Dimer test could be given to exclude the presence of a DVT. Sometimes, these tests are performed concurrently.
If after an ultrasound and an elevated d-Dimer test are done and both come out inconclusive, yet the suspicion of DVT remains high, a venogram will be given. If both tests indicate a low probability of blood clots another ultrasound may be done within a week and a follow-up after two or three months, in order to track the progression of any clots.
